Community Health and Social Care Partnership

Community Health and Social Care Partnerships (CHaSCPs) are about delivering better services and outcomes for local people, improving health and reducing health inequalities. They do this by developing closer working relationships between with partner agencies and by effective public involvement.

The CHaSCP is accountable to NHS Western Isles and Comhairle nan Eilean Siar through the Management Team, CHaSCP Committee and the Health and Social Care Committee with input from a variety of professional, voluntary and public groups.

Services provided by the CHaSCP include primary care services at various locations throughout the Western Isles, such as General Practices, Dental Services and Community Nursing.  Most people are aware of the range of diagnostic and treatment services offered by their GP and Community Nurses. They are also supported by a wide range of specialist nursing services such as Diabetes, MacMillan, Heart Disease, Multiple Sclerosis, Tissue Viability/Continence and Stroke Liaison. Screening for conditions such as cardiovascular disease are also provided through the Well North Service, adding to our prevention programmes. Public health Nurses, including Health Visitors and School Nurses, provide a full range of children’s services, including child protection.
