VACCINATION UPTAKE SURVEY

NHS Western Isles is keen to hear from people in the Western Isles who are eligible for the flu and/or COVID vaccines, to help understand what might prevent or make it difficult for people to attend for vaccination. We would appreciate if you took 5 minutes to complete the following survey, to help us improvement access.
