What is Future Care Planning?

Future Care Planning helps you make informed choices about how and where you want to be treated and supported in the future.

It requires health and care practitioners to work with you and your carer(s) to ensure the right thing is done at the right time by the right person to achieve the best outcome.

Future Care Planning puts you at the centre of the decision-making process about your health and care needs.

ReSPECT

What is the ReSPECT process?

The ReSPECT process creates personalised recommendations for your clinical care and treatment in a future emergency in which you are unable to make or express choices.

These recommendations are created through conversations between you, your family, and your health and care professionals to understand what matters to you and what is realistic in terms of your care and treatment.
