TEAM/SERVICE DESCRIPTION

Overview:

The aim of the Rheumatology Liaison Nursing Service is to provide high quality rheumatology care to its patients within a supportive and safe environment.

Target group:

The service is offered to those living and working within the Western Isles.

Services offered:

The Rheumatology Liaison Nurse Service offers a range of treatments and you may also be referred for further investigations

Location:

Clinics can be offered at the Outpatient Department at Western Isles Hospital.

In addition, we also offer

  • telephone consultation
  • video-conference consultation.
Referral method:

Patients can be referred to the Rheumatology Nurse Specialist when a diagnosis has been made by their Consultant Rheumatologist and an agreed treatment plan is in place.

Referrals can also be made by a GP or Allied Health Professionals via SCI-Gateway only.
