Integrated Front Door Service

The Integrated Front Door (IFD) service provides hospital-level urgent assessment, treatment and support to patients safely within their own home. This can improve the overall patient experience – with many patients preferring to receive treatment at home, whilst reducing unnecessary hospital admissions.

The Integrated Front Door service includes the following teams:

• Hospital at Home (H@H): Provides virtual and in-home clinical care so patients can avoid staying in a traditional hospital ward.
• Urgent & Unscheduled Care (U&UC): Manages immediate health needs and out-of-hours support.
• Frailty Practitioners: Supports older adults living with frailty.
• Acute Assessment Unit (AAU): Evaluates patients quickly to direct them to the most appropriate care path. 

In addition, the service works in partnership with many other agencies and services. This includes GPs, Community Nursing, Scottish Ambulance Service, Comhairle nan Eilean Siar, and the voluntary sector.

By working together as one service, the teams can help patients receive the right care, from the right team, in the right place as early as possible.
