Overview: | The Post Diagnostic Support service offers support for people who have been newly diagnosed with dementia. The service aims to help you self manage symptoms, encourage independence and plan for the future. People usually stay with service for a year and a named worker will manage your case. |
Target group: | People who have been diagnosed with dementia within the last 12 months |

Location: | Clinics can usually be offered at patient homes. |
Referral method: | You should be offered referral to this service by your diagnosing clinician. If this has not been offered referral can be made by your GP, social worker or other health and social care professional. |

Overview: | The memory clinic service aims to provide timely and appropriate assessment of people who are experiencing deterioration in their memory and / or thinking. |
Target group: | Any person experiencing problems with memory and / or thinking who is registered with a participating surgery. |

Location: | The clinics are held in the following Western Isles GP surgeries.
The intention of the clinics is to provide early assessment for dementia in an environment familiar to the patient. |
Referral method: | If you, or a family member or friend are experiencing memory difficulties please contact your GP initially so they can give you a health check to rule out any treatable causes for the cognitive impairment. Once this is done the GP will put you in the next available appointment for the memory clinic at their surgery. |
