The following services are provided throughout the Western Isles and offer a range of specialist nurse-led support and advice from diagnoses to support after diagnosis.

The Dementia Nursing Team provide support to people who have been diagnosed with dementia. This role is aimed at supporting and signposting people within the first year of their dementia diagnosis, and their family/carers in the community.

Dementia nurses work alongside people and their families when things get challenging or difficult giving them one-to-one support, expert guidance and practical solutions. The dementia expertise and experience nurses offer helps families to live more positively with dementia in the present, and to face the challenges of tomorrow with more confidence and less fear. The nurses have a key role to play in helping people with dementia remain in their homes, and supporting their family members and carers.

Dementia nurses have a high level of assessment skills in the observation of the patient and the surroundings in which they live and are able to identify the complexities of care needs from both a clinical and emotional perspective.

Dementia Diagnosis Clinic

This role is aimed at those who are worried about their memory and thinking. The service provides assessment for and diagnosis of dementia in GP surgeries.

Access

Access to the service is via patient's own GP. The GP will arrange the appointment to be held within their surgery and an appointment will be allocated shortly thereafter.

Post-diagnostic Support

This role is aimed at supporting and signposting people within the first year of their dementia diagnosis, and their family/carers in the community.

Referral to Post-diagnostic Support

Referrals are accepted from a GP, Consultant, Dementia Clinical Nurse Specialist, or other clinician. In all cases, referrals should be fully discussed with the patient.
