Bringing our Integrated Front Door (IFD) teams together helps create a smoother patient journey, with services working together to identify the most appropriate care for each person. 

Where appropriate, Hospital at Home can also allow people to receive hospital-level care in familiar surroundings, avoiding an unnecessary hospital admission or supporting an earlier return home. 

In simple terms, IFD is about:

  • getting you to the right service
  • providing the right care
  • involving the right healthcare professionals
  • providing care in the most appropriate place – the right place.
Impact & Statistics

Our impact: The last 6 months at-a-glance

Our Integrated Front Door services are helping more people receive the right care, in the right place, at the right time. By working across community, hospital and emergency services, we can support people closer to home where this is safe and appropriate.
  • 🚑 82 A&E attendances avoided: 82 patients attended by the Scottish Ambulance Service (SAS) did not need to attend A&E.  Working with the IFDS they were able to be appropriately supported and managed at home.
  • 🏠 1563 days of acute hospital-level care delivered at home: 1563 days of acute care were provided through Hospital at Home. This means patients received the treatment, monitoring and support they required at home, rather than occupying a hospital bed.
  • 👥 97 Comprehensive Geriatric Assessments: 97 patients received a Comprehensive Geriatric Assessment (CGA), helping to identify their individual needs and initiate appropriate referrals and further support.

Care closer to home

These figures demonstrate how our services are working together to provide alternatives to traditional hospital care. By identifying the most appropriate pathway for each person, we aim to:
  • avoid unnecessary journeys and attendances at A&E
  • prevent avoidable hospital admission
  • deliver acute care in people's own homes where appropriate
  • identify frailty and wider care needs earlier
  • connect people with the right services and support
  • help reduce pressure on hospital services and inpatient beds.
The aim is simple: wherever possible, people should receive high-quality care in the place that is most appropriate for their needs.
