What teams are involved in the Integrated Front Door (IFD) service?

• Urgent and Unscheduled Care – The Urgent and Unscheduled Care team provides urgent care in the community, including GP-requested visits during the day and responding to NHS 24 calls overnight. 

• Acute Assessment Unit (AAU) – The AAU provides assessment for patients referred for urgent hospital assessment who do not require emergency care.  (All Emergency cases would be directed through the Emergency Department)

• Hospital at Home (HaH) – Hospital at Home provides acute hospital-level care in a person's own home or homely setting when it is safe and appropriate to do so. 
  
• Frailty Practitioners: Supports older adults living with frailty.

In addition, the service works in partnership with many other agencies and services. This includes GPs, Community Nursing, Scottish Ambulance Service, Comhairle nan Eilean Siar, and the voluntary sector.
