How am I referred to the Integrated Front Door service?

Patients do not normally need to decide which Integrated Front Door team they require themselves as a healthcare professional can refer or direct a patient to the most appropriate part of the service.

Suitable patients can be referred from GPs, Urgent and Unscheduled Care, hospital doctors/consultants, the Emergency Department, AAU or an inpatient ward.

The IFD team can then help determine the most appropriate place for assessment and treatment for the patient.
