Can I refer myself to the Integrated Front Door Service (IFDS)?

IFDS is primarily accessed following assessment or referral by a healthcare professional. It will help identify the most appropriate service for you. 

Does this mean I have to come into hospital?

Not necessarily. Depending on your condition, you may be assessed at the hospital or may be suitable for treatment at home through Hospital at Home. 

What is Hospital at Home?

Hospital at Home provides acute hospital-level care at home for people who can safely be treated there. 

Is Hospital at Home only for older people?

No, the service is not limited to frail or older people. It can support patients who can safely receive the required treatment at home, subject to the service's clinical criteria. 

What happens if I become more unwell while receiving Hospital at Home care?

Patients receiving Hospital at Home care are given a contact number should they become unwell, and arrangements can be made for patient transfer to hospital, if necessary.

Will I still receive hospital-level care at home?

Yes. Hospital at Home can provide treatments and monitoring that would otherwise require hospital care, where these can be delivered safely at home.
